If you live within the area covered by the practice, please complete a registration form online or pick up a paper copy from our reception. If your application is accepted you will be registered with the practice.

Registration Forms
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
You can now complete your New Patient registration forms online.
Please ensure you are in the practice catchment area (see map above).
You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:
• passport
• birth certificate
• HC2 certificate
• rough sleepers’ identity badge
• hostel or accommodation registration or mail forwarding letter
If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address
If you prefer, you can still pick up a paper registration form from our reception.

Car Parking
Parking is available adjacent to the surgery (entrance off Brook Street). There is also limited kerbside parking at the front of the surgery but please note this subject to time restrictions. Please check the signs before you leave your vehicle as we cannot be held responsible for any penalties incurred.
Disabled Access
The surgery and toilets are designed to allow easy access for wheelchairs. An induction loop system is fitted at reception for those with hearing aids and a portable system is available for use in the consulting rooms on request.
